Financial Policies of Our Practice

Payment for Services

You are responsible for payment for medical services we provide to your family. We accept cash and checks as well as Visa, MasterCard, Discover and American Express cards. There is a $25.00 service fee added to your account if your check is returned unpaid from the bank for any reason. If you would like to pay or ask a question about your bill, please call our accounting desk at 408-356-1723.

Co-Payments Required at Time of Service

If your insurance policy requires a co-payment for physician services, we require that payment at the time of your appointment.

Have Medical Insurance?

Our practice accepts payments from most major insurers including Aetna, Anthem Blue Cross, Blue Shield, Cigna, HealthNet, United Healthcare, and other carriers.

No Medi-Cal Coverage

Please note that we do not accept Medi-Cal insurance.

Proof of Insurance Required

If you provide accurate and complete insurance information and show us your insurance card, we file our charges with your insurer as a courtesy and convenience for you.

No Insurance Card?

If you don't have your insurance card with you, we require payment at the time of services. We will give you a copy of your bill so you can file it with your insurance company.

No Insurance Policy?

If you do not have medical insurance, you must pay for our medical services at the time we provide them.


We are members of Santa Clara County Individual Practice Association (SCCIPA), a network of more than 800 physicians. If you select SCCIPA as your medical group you must have one of our physicians listed as your child's Primary Care Physician prior to your appointment. If you are new to the Bay Area or have a newborn, contact SCCIPA at 800-993-1993 to designate one of our doctors as your child's Primary Care Physician.
